The Free Fix Voucher program is for pet cats and dogs. The program provides a voucher for you to spay and/or neuter pets in your household for FREE. Since the program's funds are limited, we ask that these vouchers be used by pet owners who can not otherwise afford this surgery for their pet(s).
To qualify for the Free Fix, you must either:
1. Be on one or more of the following financial assistance programs:
Medicaid, CHIP, Food Stamps, WIC, SSD, or Section 8 (HUD), Family Employment Program (FEP), Free School Lunch, Head Start, HEAT Assistance, Supplemental Security Income (SSI), Unemployment Insurance (UI) and Veterans Benefits.
or
2. Have a household income not exceeding 200% of federal poverty level guidelines. See the following chart for qualifying incomes:

When applying for a Free Fix voucher, you must include proof of your income status (e.g. a copy of last year's tax return) and/or proof of the assistance program you receive. After our office approves and processes the application, we will send you a voucher which can be used at a participating veterinarian, on our Big Fix mobile clinic or at our Utah County Spay & Neuter Clinic.
If you don't qualify for enrollment in any of the general assistance programs and/or your income is just above the federal poverty guidelines listed above, you may qualify for a voucher at a low co-pay.
